Utah Business Registration*
Please upload a document showing the official name of the business as registered with the Utah Division of Corporations. This may include articles of incorporation, certificates of organization, certificates of existence, etc.
Floor Map
Must show where alcohol is to be served, consumed, and stored.
Cash, corporate, or surety bond
Must be in the penal amount established by the Utah Alcoholic Beverages Control Act.

Coverage amount must meet the requirements of the Utah Alcoholic Beverages Control Act.

Authorization
All information must be accurately completed or the issuance of a license will be delayed. It is a Class “B” Misdemeanor to own or operate a business in Draper City without a current Business License. I/we hereby agree to conduct said business strictly in accordance with the laws and ordinances covering such business, and swear under penalty of law that the information contained herein is true. Draper City shall not be responsible for delays in processing an incomplete application. The application you are submitting is subject to the provisions of the Utah Government Records Access and Management Act (GRAMA). If you are an “at-risk government employee” as defined in Utah Code Ann. § 63G-2-303, please inform the city employee accepting this application.
Additionally, I, the applicant, give unrestricted right to any authorized representatives of the Alcoholic Beverage Control Commission, City, or County Health Department to enter the premises where alcohol is being sold, stored, and consumed as specified in this application.
